This week, for the first time, one of our provider customers (Signature Health's Brockton Hospital in Brockton, MA) will use iGetBetter for multiple conditions.
Brockton Hospital has been using iGetBetter to guide and monitor outpatients with Congestive Heart Failure (CHF) since last February within their Homeward Bound program, using the iGetBetter CHF Care Plan, which they have tweaked for their specific needs. This program has been very successful, and they are now extending their program to include patients suffering from hypertension, using the iGetBetter Hypertension Care Plan (part of the library of standard care plan templates).
Similarly, Tallahassee Memorial Health, which has been using iGetBetter to help them manage CHF outpatients, is adding patients with diabetes. The diabetes care plan will include the monitoring of blood sugar level data from glucometers (either self-reported data if the patient wishes to continue using their current glucometer, or a Blue Tooth enabled glucometer integrated with iGetBetter for automatic data reporting).
This marks a major milestone for iGetBetter, as we extend the use of our care plans to patients with multiple conditions within the context of a single provider. Our vision is to provide a unified care coordination platform for all departments that track and manage outpatients, and this is an important step in that direction.
