Conference to Focus on Improving Care Transitions and Reducing Readmissions

Waltham, Mass., June 5, 2014 – iGetBetter, Inc., a supplier of post-acute care transition solutions, announced today that it will sponsor and exhibit its solution at the Massachusetts Hospital Association 2014 Annual Hospital-Physician Leader Conference: Engaging Physicians in Improving Care Transitions and Reducing Readmissions, to be held on June 19 in Burlington, MA.

Demonstrating CHF Care Plan & Seeking Other Pilots
The iGetBetter team of clinicians, technologists and health organization experts is conducting pilots for congestive heart failure (CHF) and total knee replacement (TKA) patients in some Massachusetts hospitals, and will be demonstrating the CHF care plan at the conference, as well as showcasing results.
In addition, the company is actively seeking additional pilot opportunities for patients with conditions such as acute coronary syndrome (ACS), cardiac artery bypass graft (CABG), chronic obstructive pulmonary disease (COPD), joint replacement and other high volume or high expenditure conditions.
