Waltham, Mass., August 13, 2014 – iGetBetter, Inc., a supplier of post-acute care transition solutions, announced today the addition of Tallahassee Memorial Healthcare (TMH), a private, not-for-profit health care system serving 17 counties in North Florida and South Georgia, to its roster of hospitals piloting its flagship care transition platform. The iGetBetter cloud-based platform bridges transition of care from hospital to home and engages patients in their own care, thus improving outcomes and helping to reduce readmissions.
In 2011, in partnership with the Florida State University College of Medicine and Capital Health Plan, TMH created the Transition Care Center with the mission to reduce healthcare cost by lowering readmissions in the Leon County in Florida.  The establishment of the Center enabled the hospital and the community to collaborate on population health management and has already impacted more than 5,000 patients.  Remote patient monitoring allows the Center to intervene and assist the patient while they are at home, which reduces visits to the emergency room and the hospital, and allows patients to gain a better understanding of how to manage their own health.
“The initial response from the high-risk patients we enrolled in the CHF pilot of the iGetBetter platform has been very positive,” said Lauren Faison, administrator at the Tallahassee Memorial Healthcare Transition Center. “The patients found the platform easy to use and are engaged to communicate their health status with both their medical providers as well as their family.”
TMH Transition Care Center Achieving Early Triple-Aim Goals
With the new risk-sharing payment models, providers need rapid deployment of care transition solutions that can help them engage and monitor their patients.  Many hospitals trying to achieve a better population health standard have undergone the Institute for Healthcare Improvement’s Triple-Aim Initiative collaborative training.
“It is not surprising that a hospital which is part of IHI’s Triple-Aim initiative is leading the use of technology to help achieve the three goals of better patient experience, quality outcome and lower cost,” said David Lebudzinski, M.D., chief medical officer at iGetBetter. “In this CHF pilot, the TMH Transition Center’s team identifies the best patients suited for self-health management and provides them with the blood pressure cuff and scale, as well as an overview of how to use iGetBetter for interaction with their clinical team as well as their family members.”
The TMH Transition Center has implemented several innovative strategies including the use of telemedicine and remote patient monitoring, and has embraced transitional care as an effective means for ensuring that patients receive the necessary care they need to heal well and ultimately live well after a hospital stay. The multidisciplinary team comprised of a physician, pharmacist, nurse, and social worker work collaboratively to ensure that patients can access follow up treatment and ongoing care. These efforts have already proven to be successful in reducing costs and improving outcomes.

Seeking Pilots
The iGetBetter team of clinicians, technologists and health organization experts is conducting pilots for congestive heart failure (CHF) and total knee replacement (TKR) patients in various hospitals.  In addition, the company is actively seeking additional pilot opportunities for patients with conditions such as acute coronary syndrome (ACS), cardiac artery bypass graft (CABG), chronic obstructive pulmonary disease (COPD), joint replacement and other high volume or high expenditure conditions.
